In order to protect the safety and health of all workers in the Company and prevent occupational disasters, this Safety and Health Code of Practice is established in accordance with Article 34 of the Occupational Safety and Health Act for employees to comply with and is announced to be implemented.
1.Safety and health management of the business and the authority and responsibility of each level
(I) The safety and health management of the Company shall be administered by the person in charge or its acting person, and the supervisors at each level shall direct and supervise the employees under their supervision according to their authority. (II) The Company's employees shall strictly comply with the following. 1. The Safety and Health Code of Practice of the Company. 2. Safety and health matters should be followed as instructed by the person in charge of the Company or supervisors. 3. Participate in the safety and health education and training held or assigned by the Company.
2.Maintenance and inspection of machinery, equipment or apparatus
(I) The regular inspection and highlight inspection of each machinery and equipment shall be recorded and kept for three years for the following items: : 1. Inspection year, month, day. 2. Inspection method. 3. Inspection part. 4. Inspection results. 5. Inspector name. 6. The content of improvement measures to be taken based on the inspection results. (II) For general vehicles, the user shall check the vehicle for abnormalities before the daily operation according to the regulations, and the Company shall regularly inspect the vehicle's safety performance once every three months and record it. (III) The Company shall regularly inspect and record the high and low voltage electrical equipment once a year in accordance with the regulations. (IV) The Company shall regularly inspect the lifts once a year in accordance with the regulations on the machinery as a whole. The Company shall regularly inspect and record the aforementioned lifts once a month in accordance with the regulations. (V) The Company shall inspect the aerial operating trucks and stackers annually or monthly according to the stipulated items, carry out the relevant inspections of the aerial operating trucks during daily operations, and record them. (VI) The Company shall complete the qualification inspection of the dangerous machinery and equipment installed and used before operation, conduct regular self-inspections annually or monthly, conduct inspections before and after use or when there are abnormal conditions, and record them. (VII) The Company's cranes, pressure vessels, small boilers that are not classified as dangerous machinery and equipment shall be regularly inspected and recorded every year. (VIII) If any malfunction or abnormality is found in the equipment, machinery, apparatus, property or articles in charge or under custody that may endanger health or safety, the use of such equipment, machinery, apparatus, property or articles shall be stopped and reported to supervisors or the secretariat, and the operation shall be started only after repair or confirmation of normalcy. 3. Work safety and health standards
(1.) General safety and health precautions. (I) Safety and health should be given priority in any operation. Employees should remind each other to pay attention to safety, supervisors or administrators should pay attention to the physical and mental conditions of employees. In case of unsafe conditions, employees should be asked to correct immediately. (II) Before going to work or during work, it is strictly forbidden to drink alcohol or abuse drugs that are harmful to the body and mind or disturb the spirit. Supervisors or administrators should also pay attention to the physical and mental condition of employees at all times. (III) When there is an immediate danger in the workplace, the employer or the person in charge of the workplace shall order the work to stop and the laborers to retreat to a safe place. (IV) If employees feel unwell or in a bad mood at work and cannot perform the assigned work, they should report to their supervisors or administrators for reassignment or leave of absence. (V) If employees are unable to perform the assigned work, they should clearly indicate to their supervisors that they are unable to perform the assigned work, and do not take any risks that may harm themselves or others. (VI) Chasing, playing, flirting or pranks are strictly prohibited in the workplace. (VII) The workplace should be kept clean and sanitary, and no one is allowed to smoke, chew betel nut, throw cigarette butts, confetti, or spit betel nut juice at will. (VIII) For indoor workplaces that are significantly hot and humid, cold, and hazardous to workers' health, appropriate air conditioning facilities such as an air conditioner, heater, or ventilator should be installed. (IX) For the workplace, lighting equipment should be set up in accordance with the regulations to maintain appropriate lighting. (X) Operations should be in accordance with standard operating procedures and shall not change the operating method without authorization. (XI) For changes in operating procedures or unexpected events and other non-routine operations, contact the relevant departments in advance and understand the work procedures, safe working methods, related operations and related matters. Supervisors or relevant units should send staff to supervise or coordinate when necessary. (XII) The shields, fences, grounding and other safety devices of apparatus, machinery and equipment shall not be damaged or rendered ineffective, and if it is found that the safety device is damaged, immediately report to supervisors or the relevant units. (2.) Prevention of material stacking and transport disasters. (I) For material transport, all items over 40 kg will be transported by rickshaws or tools, and items over 500 kg will be transported by mechanical vehicles or other machinery. (II) When using machinery and apparatus to transport goods, employees should abide by the Company's planned speed limit and drive in accordance with the planned road. Make sure to turn on the lighting equipment of the transporting machinery. When approaching a road intersection, slow down and make sure there is no risk of collision at the intersection before continuing to transport. (III) The stacked materials should be bundled with ropes provided by the Company. A protective net should be added and piles should be set accurately. Restrict the stacking height or change the way of stacking, etc. Ensure to comply with the Company's material stacking strategy. (IV) Other than operators, other unrelated personnel are not allowed to enter the site. (3.) Prevention of hazards of fall and ladder use. (I) At the site where there may be a collision or it is possible to be hit by flying or flying objects during supervisory operations with a height of more than two meters, on-site operators, supervisory personnel, etc. should properly wear helmets with proper headbands securely tied. (II) Other than operators, other unrelated personnel are not allowed to enter the site. (III) When supervising operation at the edges and openings of workplaces that are not equipped with platforms and guardrails and are more than two meters above the ground, on-site operators and supervisory personnel should properly use safety belts and supplemental ropes or vertical (horizontal) ropes, and other necessary fall prevention facilities. (IV) When working on roofs and rain shelters constructed of easy-to-step-through materials such as a sponge board, tin plate, tile, wood, thatch, plastic, etc., or on mezzanine ceilings constructed of easy-to-step-through materials such as mineral fiber board and gypsum board, the following facilities should be adopted. (1) The roof frame, rain shelter or ceiling support shall be equipped with a footboard of sufficient strength and width of 30cm or more provided by the Company as a safe passage. (2) Work in two-person teams, with one person supervising the operation at all times. (V) Workplaces with a height of more than 1.5 meters should be equipped with safety equipment for getting up and down. (VI) The following requirements should be confirmed when using a moving ladder to move up and down: (1) It should be confirmed that the structure is solid and that the material is not significantly damaged or corroded, and that the tread width is more than thirty centimeters. (2) The necessary measures should be taken to prevent slippage or rotation. 7. The trestle ladder shall not be used as equipment to move up and down the two working surfaces and standing on the top plate is prohibited. The following requirements should be confirmed for the trestle ladder. (1) It should be confirmed that the structure is solid and that the material is not significantly damaged or corroded. There is a safe non-slip ladder surface. (2) The angle between the foot of the ladder and the ground should be within 75 degrees, and there is hard material such as metal between the two feet of the ladder to fasten, and there is a non-slip cover on the feet. (4.) The prevention of musculoskeletal disorders caused by repetitive operations. (I) Use of computers for work: (1) The monitor support should be able to rotate and adjust its inclination. The screen should be perpendicular to the line of sight, and the viewing angle should be about 15-20 degrees downward. (2) Comfortable viewing distance for reading texts of average size, about 350 - 600 mm. (3) The forearm is at an approximate right angle to the arm. (4) If necessary, a stable footrest can be used for the adjustable seat back and seat height, and there should be enough space to stretch with the lower limbs. (5) If necessary, set up adjustable file folders on both sides of the monitor to avoid shoulder and neck pain. (6) When using the keyboard, keep the wrist straight or at least slightly tilted, and if necessary, use a wrist pad (the height of the wrist pad should be equivalent to the height of the first line of keyboard keys; have sufficient depth to support the wrist; and should not have a sharp hard edge against the skin; made of breathable material is appropriate). This will reduce the risk of repetitive strain and injury. However, when using the keyboard to enter data, the wrist should not be pressed against the wrist pad and bent to use the keys on either side. The correct way is to move the entire hand to enter the data. The wrist should only be placed on the wrist pad during a pause in work for a rest. (7) When using the mouse, avoid holding the mouse tightly or using too much force to press the keys; avoid bending the wrist forward or to the side; and keep the mouse at the same height as the keyboard. (8) If an employee needs to operate a computer and answer the phone simultaneously for a long time, he/she should use a headset and avoid holding the receiver by the shoulder and neck. (9) Desktop lighting luminosity should be between 300 to 500 LUX. (10) Keeping the same posture for a long time may lead to muscle strain, so you should change posture or do other work from time to time. After 1 to 2 hours of computer work, take 5 to 15 minutes to do something to let the eyes rest, or stand up to stretch the body and look at the distance. (II) Relaxation exercises: The following suggested exercises aim to intermittently move various parts of the body to help eliminate fatigue and prevent musculoskeletal disorders, and each exercise can be repeated several times, but if you feel really uncomfortable, it is appropriate to seek medical attention as soon as possible. (1) Eye exercise: 1. Straighten the body, fix the head, look upward and then downward at the ground. 2. Slowly turn the eyes from side to side and look at the objects on the left and right. 3. Look at the objects in the upper right and lower right corners, then the objects in the upper left and lower left corners. (2) Neck exercise: 1. Arms hang down naturally at the sides. Gently tilt the head forward and stretch the back of the neck. Pause for 5 seconds. 2. Arms hang down naturally at the sides. Twist the head to one side and then to the other side. Pause for 5 seconds on each side. 3. Arms hang down naturally at the sides. Swing the head back and forth to the left and right. Pause for 5 seconds on each side. (3) Shoulder exercise: Lift both shoulders upward, then slowly turn backward, repeat 10 times. (4) Upper limb exercise: Cross fingers with both hands, stretch them straight up, and hold palms upside down. Pause for 10-15 seconds. Then relax your shoulders. Remember to breathe deeply during the exercise. (5) Upper back exercise: Upper back exercise: Sit down first, cross fingers at the back of your head, stretch your elbows out, and straighten your upper body. Squeeze both shoulder blades inward so that the upper back and shoulder blades feel pressure. Pause for 5 seconds and then relax. (6) Hand exercises: 1. Open fingers as much as possible, pause for 10 seconds; then relax. 2. Stretch both hands forward, palms down, and slowly bend wrists upward (palms forward). Pause for 10 seconds. Then slowly bend downward (palms forward, pause for 10 seconds). (III) Prevention of physical or mental abuse in the performance of duties due to the actions of others. (1) No alcoholic beverages should be consumed during working hours. Those who take medication should follow medical advice to use the medication correctly and fully understand the possible side effects. If necessary, inform supervisors, relevant administrators or colleagues. (2) Employees should get along with each other harmoniously and should fully communicate and work together to resolve conflicts at work through mutual respect. If there is still a dispute, it should be decided or handled by the officer in charge according to the regulations and duties. Violence, verbal abuse, defamation, coercion, harassment, and intimidation shall not be used against others. The persecuted person may seek help through the complaint or judicial channels, and the relevant supervisors or administrators shall provide necessary assistance. (3) Perform duties in accordance with the prescribed procedures and pay attention to personal attitude and tone of voice to avoid conflicts. (4) Supervisors or administrators shall deploy sufficient manpower in a timely manner to assist employees in carrying out their duties, depending on the status of their duties. (5) Upon discovering any difference in another person's behavior, speech, physical or mental condition while performing duties, the employee shall immediately suspend or terminate the duties and report to supervisors or administrators for their knowledge. (6) Upon discovering that the behavior of others may endanger personal safety while performing duties, the employee should immediately evade and take the necessary emergency response and protective measures. The Company shall provide and train employees to use emergency response and protective measures as needed in accordance with relevant regulations. (7) If the behavior of employees or others endangers the safety of the personnel or the Company, the Company shall immediately notify the police and fire-fighting units to deal with the situation, and if necessary, notify other personnel to evacuate and take refuge. The relevant authority of the Company shall direct and respond to the situation in a timely manner. (IV.) Evacuation, first aid, rest or other matters to protect the physical and mental health of employees. (1) Company office entrances and exits, passageways, corridors, stairs, emergency exits, emergency lighting, drainage, escape and firefighting and other emergency evacuation equipment should be set up according to regulations with regular inspection and maintenance annually; no one shall damage, block, and lock them, pile up miscellaneous objects and conduct other acts affecting their normal functions, and when abnormalities are found, the secretariat and administrative unit or administrators should be informed immediately to deal with it. (2) The Company should have first aid medicine and equipment: Sterilized gauze, sterilized cotton, tourniquet, adhesive tape, triangular towel, regular scissors, straight tweezers, splints, bandages, safety pins, superior iodine, and other necessary medicine, and the secretariat and administrative unit should check for their completeness and replenish them at any time. (3) Maintain the safety and environmental hygiene of the office building and equipment, be clean and tidy, and avoid noise and damage. (4) All employees should follow the Company's standard operating procedures for emergency evacuation and accept the Company’s office fire and related disaster simulation drill held once every 2 years. (5) All employees can report safety and health protection issues at any time, and the Company, supervisors or administrators at all levels shall assist, understand and make necessary arrangements. The secretariat and administrative unit shall establish and update the emergency notification and contact telephone information regularly. (6) In case of immediate danger in the workplace, all operations shall be stopped immediately and employees shall be evacuated to a safe place. (7) Employees performing duties found to be in immediate danger can stop work and retreat to a safe place without endangering the safety of other workers. (8) For female employees during pregnancy or less than one year after childbirth, necessary work adjustment or health protection measures shall be taken according to the recommendation of the physician's suitability assessment. During the period of personnel protection mentioned above, the Company shall take into account the recommendation of the physician and take appropriate disposal measures according to the relevant laws and regulations after the assessment of the physician to confirm the unsuitability of the original worker due to changes in working conditions, operating procedures, abnormal health of the employees concerned or uncomfortable reactions. (9) On typhoon days when the local government has announced the suspension of work, field work shall be suspended. For duties assigned by the Company, the Company shall provide appropriate protection depending on the hazard of the work, and prepare appropriate life jackets, helmets, communication equipment and other necessary safety protection facilities and transportation. (10) For employees working outdoors during the summer season, to prevent heat illness caused by a high temperature environment, employees should pay attention to sun protection, hydrate every 15 minutes, and rest in a shady place at the right time. When the initial symptoms of heat illness such as headache or weakness are found, they should rest and hydrate in a shady place as soon as possible, and if they still have uncomfortable symptoms, call 119 for assistance. Supervisors or administrators should assign work according to the weather conditions and allow sufficient rest time. 4. Education and Training

(II) The Company, supervisors or administrators shall provide education and training to employees who perform various operations in accordance with the regulations so that they have the necessary ability to perform their duties. (III) Personnel serving emergency care in the applicable premises shall be trained as first-aiders. (IV) New or current employees shall receive at least three hours of general safety and health education and training appropriate to the job before changing jobs; and at least three hours of safety and on-the-job health training every three years depending on the nature of the job. Other workers who are required to obtain relevant certificates or training before they can be hired shall not be employed unless they are trained and qualified in accordance with the Occupational Safety and Health Act, the Occupational Safety and Health Education and Training Rules or relevant regulations, and shall receive on-the-job education and training in accordance with relevant regulations. (V) Labor safety and health personnel should have at least six hours of on-the-job training every two years. Supervisors of specific chemical operations shall receive at least six hours of on-the-job training every three years. First aiders, members of the Labor Safety and Health Committee, and business supervisors all levels of management, command, and supervision shall receive at least three hours of on-the-job training every three years. 5. Health guidance and management measures

(II) According to the checkup results, employees shall accept the appropriate work adjustment, medical treatment, recuperation, and management. (III) Employees shall cooperate with the Company in accordance with the regulations in the arrangements related to work content, monitoring of the operating environment and investigation of hazard exposure. (IV) Employees should accept the Company's instructions and management regarding working hours, rest, epidemic prevention, injury prevention, and health promotion for physical and mental health protection in accordance with regulations. 6. First aid and rescue
(I) Those who carry out first aid and rescue should first consider their own safety and ability, do not take the risk, if necessary, and should immediately call 119 rescue. (II) Any injury (regardless of the severity) should be immediately reported to supervisors or administrators, shall not be concealed and not reported. (III) In the event of an electrical induction disaster, first try to turn off the power supply and confirm that there is no risk of electrical induction, then apply first aid and rescue. (IV) Any first aid is only to maintain the life of the injured or to avoid the expansion of the injury; for major injuries, the injured should be sent to medical institutions for further treatment. 7. The preparation, maintenance and use of protective equipment
(I) When transporting, placing, or using the thorny or protruding objects, corrosive substances, toxic substances, or highly toxic substances, appropriate protective gears such as gloves, aprons, leg wraps, safety shoes, helmets, protective glasses, anti-toxic or safety masks should be used. (II) When there is a risk of objects flying down or scattered during operation, use appropriate safety helmets, safety goggles and other protection. (III) When working at a height of 2 meters or more and there is a risk of falling, safety belts, helmets and other necessary protective equipment should be used. (IV) The railings, guard fences, lifting equipment and other devices set up at high workplaces shall not be damaged or made invalid. If damage is found, the operation shall be stopped immediately and the situation reported to the supervisor or foreman. (V) Employees engaged in electrical work shall use electrical safety helmets, insulation protection gears and other necessary protective apparatus. (VI) The Company shall provide adequate and compliant personal protective gears for employees to use, and shall train employees in the use and wearing methods, and supervisors or administrators shall supervise their use by employees. (VII) Personal protective gears should be kept clean and disinfected as necessary. They should be properly preserved and frequently inspected to maintain their performance and replaced at any time when the performance is poor or damaged. Before and after daily operations, checking is actually conducted and recorded. 8. Accident notification and reporting
(I) Any incident or accidental situation, regardless of the magnitude, whether there are personnel injuries or machinery or equipment damage, in addition to responding immediately in accordance with the authority and responsibility, report to supervisors or administrators right away. After receiving the report, the supervisor or administrator shall immediately take necessary actions or assist according to the situation and regulations. (II) In the event of one of the following occupational disasters occurring in the workplace, the supervisor or administrator shall immediately report to the superior and, except for necessary first aid and rescue, shall not move or damage the site without the permission of the judicial agency or inspection institution. The Company shall notify the local labor inspection institution within 8 hours. 1. A fatal disaster occurs. 2. The number of victims of the disaster is three or more. 3. The number of victims of the disaster is one or more, who require hospitalization. 4. Other disasters designated and announced by the central competent authority. (I) If the unit's laborers violate the provisions of this Code, depending on the severity of the case, it may be reported to the competent authorities for punishment according to law. (II) Workers who are not employed by the unit to enter and work in the unit shall abide by the relevant regulations. 10. Supplementary Provisions
This Code has been established by the relevant personnel of the unit and the labor representatives. It has been reported to the Occupational Safety and Health Administration of the Ministry of Labor for review and then announced for implementation. The same applies to revision or addition. The Company's retirement system is governed by the Labor Standards Act and the Labor Pension Act of the country, including those who have worked for over 15 years and have reached the age of 55, those who have worked for over 25 years, and those who have worked for over 10 years and have reached the age of 60 may voluntarily retire.
(I) The Company contributes 6% of the employees' monthly wages to the employees' individual pension accounts in accordance with the "Table of Monthly Contribution Wages of Labor Pension" issued by the Bureau of Labor Insurance. (II) Employee's voluntary pension contribution: Employees can also make voluntary pension contributions within 6% of their monthly wages according to their personal preference. (III) As of 2021, the Company's pension under the new system was NT$2,859,964.
